The Directorate General of Civil Aviation has formed a four-member commitee to conduct a 'comprehensive' probe into massive disruptions in IndiGo's flight operations earlier this month. On Friday, an eight-member committee, headed by Joint Director General Sanjay K. Bramhane, submitted its probe report on IndiGo's operational chaos.
According to DGCA, the four-member probe team will conduct a comprehensive review and assessment of the circumstances that led to operational disruptions of IndiGo Airlines. According to the Directorate, though IndiGo was repeatedly advised about the new guidelines under Flight Duty Time Limitations rules, the airline failed to forecast crew availability and realign rosters.
"DGCA has issued repeated directions and advance instructions from time to time to Indigo for having timely preparation to implement the provisions of the Civil Aviation Requirements," it said. The directorate said IndiGo's inability "to accurately forecast crew availability, conduct timely training, and realign rosters despite advance regulatory intimation resulted in cascading delays and cancellations across its network beginning late November 2025."
India's domestic civil aviation sector almost collapsed after IndiGo cancelled hundreds of flights on a single day, owing to new FDTL regulations.
The civil aviation authority said the airline "acknowledged" that it failed to anticipate the actual crew requirement under the revised norms during a review meeting. "This prima facie indicates deficiencies in internal oversight, operational preparedness, and compliance planning, warranting an independent examination," it said.

According to DGCA, the four-member probe panel headed by Sanjay K. Bramhane, Joint Director General of DGCA, will undertake "a comprehensive review and assessment of the circumstances that led to such massive disruption in the scheduled flights."
The probe panel will identify causes that led to widespread operational disruptions, review the extent of compliance with the revised FDTL provisions, including analysis of gaps admitted by the airline, and review the mitigation measures being undertaken by IndiGo. The committee has been directed to file its report within 15 days.
