Mohan walked into the clinic holding his right cheek, visibly frustrated. Every time he chewed his food on that side, a sharp, wincing pain shot through his jaw.
What puzzled Mohan most was the culprit tooth: it was a lower molar that had a root canal years ago. "The nerve was removed, Doc," he said. "How can a dead tooth hurt so much?"
The answer surprised him. The pain wasn't coming from a sensitive nerve inside the tooth—it was coming from the bone and gum tissue surrounding it. Mohan was dealing with a classic case of Cracked Tooth Syndrome.
Why do root canal treated teeth crack?
Think of a natural, healthy tooth like a green tree branch. It is hydrated, supple, and flexes slightly under pressure.
When a tooth undergoes a root canal, the infected nerve and blood supply are cleared out. While this saves the tooth from immediate infection, it turns that flexible green branch into dry firewood over time. The tooth becomes brittle and loses its natural shock-absorbing ability.
Every time we chew, our jaw muscles generate immense pressure—easily over 150 to 200 pounds on the back molars. If a root-canaled tooth is left with just a large filling instead of being covered by a sturdy cap (crown), those chewing forces act like a wedge splitting a log. Add common habits like night-time teeth grinding, chewing ice, or biting on hard nuts, and tiny hairline cracks inevitably begin to spread.
Why normal X-rays miss it: The CBCT difference
Mohan's previous regular dental X-ray came back looking completely normal. That is because standard dental X-rays are flat, 2D images—much like looking at the shadow of an object on a wall. If a hairline crack doesn't sit directly in the path of the beam, the X-ray simply cannot see it.

Think of a CBCT as a high-definition 3D photograph of your jaw. Instead of a flat picture, it lets a dentist rotate, slice, and inspect the tooth from every angle. While some cracks are microscopic, a CBCT reveals the telltale red flags of a fractured root: distinct, pinpoint bone loss wrapped around the root where bacteria have begun seeping through the fracture line.
Why a vertical crack means the tooth must come out
Once the 3D scan revealed a vertical root fracture—a crack running straight down through the root into the jawbone—Mohan asked if another filling or medication could fix it. Unfortunately, vertical root fractures cannot heal. Unlike a broken bone in your arm, a cracked tooth cannot knit itself back together.
Every time you chew, the two halves of the root flex microscopically. This flexing pumps saliva, food particles, and bacteria straight into your jawbone. Trying to save a vertically split root is like gluing a cracked drinking glass back together and expecting it to hold boiling water under heavy pressure. Leaving it in place only destroys more of the supporting jawbone. That is why gently extracting the tooth is the safest, most predictable option.
Prevention: Protecting your teeth before they crack
Preventing a crack is always easier than treating one.
Cap root-canaled teeth promptly: Always protect back teeth with a crown or onlay after root canal treatment to hold the tooth together like a tight barrel hoop.
Wear a nightguard: If you clench or grind your teeth while sleeping, a custom nightguard cushions the blow and protects your enamel.
Avoid biting hard objects: Skip the habit of cracking ice cubes, popcorn kernels, or opening packaging with your teeth.
Once Mohan understood why his tooth couldn't be saved, we proceeded with a gentle, careful extraction to protect the surrounding bone. After allowing the area to heal properly, Mohan received a dental implant—a biocompatible titanium post that acts as a brand-new root—finished with a natural-looking porcelain crown. Today, Mohan is chewing comfortably on both sides of his mouth again, free of pain and confident with a strong, permanent tooth.
