The deaths of actor Irrfan Khan in 2020 and Apple co-founder Steve Jobs in 2011 drew worldwide attention to pancreatic cancer. Both had a relatively uncommon form of the disease—pancreatic neuroendocrine tumour—which is generally less aggressive. Most patients develop pancreatic adenocarcinoma, a far more common and deadlier form of the disease.
Despite remarkable progress in cancer care, pancreatic adenocarcinoma has continued to be one of the most difficult cancers to treat. Nearly half a million people worldwide are diagnosed with the disease each year, and the numbers continue to rise. India is witnessing a similar trend, with increasing cases being reported across several states, making pancreatic cancer an emerging public health concern. One of the reasons pancreatic cancer is so deadly is that it is often diagnosed late. Early warning signs are subtle and easily mistaken for more common illnesses. As a result, many patients are diagnosed only after the cancer has spread beyond the pancreas, when the chances of cure are reduced.
Even with the best treatments available today, the outlook remains sobering. For patients with advanced disease, survival is often measured in months rather than years, making pancreatic cancer one of the greatest challenges in modern oncology. That is why a recent study presented in the Oncology Global meeting (ASCO) in Chicago and published in the New England Journal of Medicine has generated considerable excitement among patients and cancer researchers worldwide. The breakthrough centres on a gene called KRAS. Scientists have known for years that mutations in this gene act as a master switch driving the growth of most pancreatic cancers. More than 90 per cent of pancreatic adenocarcinomas carry abnormalities in this pathway. Yet, despite decades of research, KRAS earned a reputation as an “undruggable” target—a critical cancer driver that seemed impossible to block with medicines. That barrier may finally have been overcome with the development of a new class of targeted medicines known as RAS inhibitors.
In a large Phase III clinical trial involving about 500 patients with advanced pancreatic cancer carrying a KRAS mutation, the new targeted treatment was compared with standard chemotherapy. The results were remarkable. Patients who received the targeted drug had a median survival of 13.2 months, compared with 6.6 months among those who received conventional chemotherapy. At first glance, a gain of roughly six months may not seem dramatic. But in pancreatic cancer, where meaningful advances have been rare, doubling survival represents a significant milestone. More important, the study proves that one of the disease’s most important biological drivers can finally be targeted. Cancer medicine has witnessed similar turning points before. Targeted therapies have transformed the outlook for diseases such as chronic myeloid leukaemia and HER2-positive breast cancer. Many experts believe KRAS-directed therapies could mark the beginning of a similar transformation in pancreatic cancer and potentially in other cancers.
The implications for India are especially important. As the population ages and the burden of cancer grows, pancreatic cancer is emerging as a significant public health challenge. A treatment that improves survival and creates new therapeutic possibilities could eventually benefit thousands of Indian families. The new treatment has yet to receive widespread regulatory approval, and access remains limited, largely through clinical trials. Important questions remain about cost, long-term effectiveness and how best these drugs can be combined with surgery, chemotherapy, immunotherapy and radiation. Yet, the broader significance of this discovery extends far beyond a single medicine. It highlights how decades of fundamental scientific research can ultimately translate into meaningful advances for patients. What once seemed impossible—the ability to target KRAS—has now become a reality.
This breakthrough does not erase the challenges posed by one of the world’s deadliest cancers. But it offers something that has been missing for a long time: a credible reason for optimism. And, in the long fight against pancreatic cancer, that is no small achievement.
