In an emergency room, minutes are not just numbers on a clock. They determine how quickly a patient with a heart attack receives treatment, how soon a stroke patient is assessed, or how quickly a critically ill patient is admitted. Yet hospitals often have limited visibility into where those minutes are actually being lost.

A simple paper wristband with a QR code is offering an unlikely solution. As part of a quality improvement programme modelled on principles used in space mission operations, the system was tested across 17 hospitals in India. Ten hospitals completed a before-and-after comparison involving 10,332 patients. The findings, published in the International Journal for Quality in Health Care, showed measurable reductions in several emergency-care delays.

A timestamp for every step

The concept is simple. A patient receives a wristband carrying a unique QR code. At key stages of care registration, triage, doctor assessment, blood collection, test results and discharge a staff member scans the code using a mobile application.

Each scan takes less than 20 seconds and automatically records the time.

The value is not the QR code itself. It is the visibility it creates. Instead of relying on paper registers or staff recollection, emergency teams can see, minute by minute, how long each stage of a patient's journey takes and where the bottlenecks occur.

The results were significant. Triage time fell by about 11 minutes, pain assessment time by 23 minutes and the time taken to review test results by 26 hours. Overall disposition time the decision on whether a patient could be discharged or needed admission fell by approximately 90 minutes.

Why minutes matter in an emergency

For a patient, a delay can feel like waiting. For an emergency physician, it can represent lost treatment time.

A patient arriving with chest pain, sudden weakness, breathing difficulty or other symptoms of a time-critical emergency cannot afford an inefficient system. In conditions such as stroke and heart attack, treatment windows are limited, and delays can affect outcomes.

The important point is that the wristband does not make clinical decisions. It makes delays visible.

Emergency departments often have an idea of where their problems lie. Data can tell a different story. A department may believe that triage is the main bottleneck, only to discover that test reporting or decision-making after investigation is taking longer. Once these delays become measurable, teams can begin addressing them.

Patient safety does not always require expensive technology

Health care improvement is often associated with new equipment, sophisticated software and large investments. Those have an important role, but patient safety can also begin with something much simpler: knowing exactly what is happening.

The QR wristband uses basic technology like a printed code, a smartphone and real-time timestamps to create a clearer picture of emergency care.

For India's public and mid-sized hospitals, where resources can be constrained, this is an important lesson. Improving emergency care is not always about adding another machine. Sometimes it is about removing the uncertainty around the machines, people and processes already in place. The biggest lesson is therefore not about QR codes. It is about accountability to time.

In an emergency room, every minute should have a purpose. If a hospital can identify where those minutes are being lost, it has already taken the first step towards giving them back to the patient.
