In a recent Instagram reel posted by 'animationplusplus', a page with 5.66 lakh followers, an animated video, which has garnered 6.68 lakh views, explains what varicose veins are and how they should be managed.
Through the animation, it is explained that varicose veins occur because "the valves of the veins in the feet have broken. The blood is flowing backwards and is pooling." The video stresses that "No cream will cure it," and advises people with varicose veins to wear compression stockings to help relieve symptoms. However, it specifies that "wear them only before getting up in the morning." It also recommends walking or doing seated calf raises, explaining that "Calf muscles pump blood upwards." The reel further advises people not to stand in one place for long hours, to elevate their legs above heart level during the day, and cautions viewers, "Don't trust apple cider vinegar. Always consult a doctor before taking supplements."
The caption of the post adds that "Varicose veins aren't just a cosmetic issue. They're a sign that the veins in your legs are struggling to push blood back to your heart." It explains that weakened or damaged vein valves allow blood to pool in the legs, while risk factors include prolonged standing or sitting, pregnancy, obesity, ageing, family history and physical inactivity. It also recommends regular walking, maintaining a healthy weight, elevating the legs after a long day and staying physically active through exercises such as walking, cycling or swimming. The caption further advises people to "Wear compression stockings if recommended by your doctor," and urges those experiencing severe pain, swelling, skin changes, bleeding or ulcers to consult a vascular specialist, noting that "modern treatments are effective and often minimally invasive."
Do compression socks help alleviate varicose veins?
Research suggests that compression stockings can help relieve symptoms associated with varicose veins, but the extent of their benefit varies depending on the patient, the stage of disease and the treatment setting. While several studies report improvements in pain, swelling and heaviness, they do not suggest that compression stockings reverse the underlying damage to the veins.
Evidence from a 2014 randomised trial compared conservative treatment with compression stockings against surgery in 153 patients with superficial venous reflux. Although patients using compression stockings experienced a modest improvement in symptom severity, those who underwent surgery showed substantially greater reductions in disease severity scores and significantly better health-related quality of life after two years. The researchers concluded that "the surgical elimination of non-complicated superficial venous reflux is an effective treatment when compared with providing compression stockings only."
The evidence surrounding compression after varicose vein procedures also remains uncertain. A 2015 systematic review analysed seven randomised controlled trials evaluating different compression strategies following surgery, foam sclerotherapy and endovenous laser ablation. While some individual studies suggested short-term benefits—such as less pain after seven days of stocking use following laser treatment—the studies varied considerably in design, duration and type of compression used. Because of this heterogeneity, the authors concluded that "there is currently little quality evidence upon which to base any recommendations concerning compression following treatment for varicose veins."
More direct evidence on symptom relief came from a 2018 randomised, double-blind, placebo-controlled trial involving 30 patients with primary varicose veins. Participants wearing graduated elastic compression stockings (18–21 mmHg) reported significantly less leg pain and aching after one week than those wearing placebo stockings. They also showed favourable trends in swelling, paraesthesia and other symptoms, although these differences were not statistically significant. The authors concluded that "graduated elastic compression stockings seem effective in ameliorating symptoms, particularly pain or aching, compared with placebo stockings after one week of use," while noting that longer-term studies are warranted.
More recently, the 2023 International Union of Phlebology (UIP) One-Page Guideline described compression therapy as "an indispensable treatment option" in the management of chronic venous disease. According to the guideline, graduated compression stockings support the calf-muscle pump, reduce venous hypertension and improve venous and lymphatic return. They are recommended to improve symptoms, reduce leg swelling and help improve quality of life in patients with symptomatic varicose veins and other stages of chronic venous disease. However, the guideline also points out that evidence on whether compression slows disease progression remains limited and notes that long-term compliance is often poor because patients may find stockings uncomfortable or difficult to use consistently.
Supporting these recommendations, a 2024 randomised controlled trial involving 50 patients with uncomplicated varicose veins (CEAP C2s–C4a) found that wearing Class I medical compression stockings for four weeks significantly reduced overall symptom scores compared with no treatment. Patients in the compression group experienced significant improvements in pain, leg swelling and the feeling of heaviness, whereas symptoms remained largely unchanged in the control group. The researchers concluded that medical compression stockings "significantly improve the frequency and severity of symptoms in patients with uncomplicated varicose veins."
The recommendations are also reflected in guidance from the NHS, which states that compression stockings work by applying pressure to the lower legs, thereby improving blood flow and helping reduce swelling and pain. However, the NHS notes that they are generally offered when procedures or surgery are not suitable, or while waiting for treatment, rather than as a cure for varicose veins. It also advises wearing them during the day and removing them at bedtime, while noting that pregnancy-related varicose veins often improve after childbirth.
What experts say
According to Dr Krishna Chaitanya, Lead Consultant - Vascular and Endovascular Surgery, Aster CMI Hospital, Bengaluru, compression stockings are among the most commonly recommended first-line treatments for people with varicose veins because they apply gentle pressure to the legs, improving blood flow and reducing symptoms.
"They are effective in reducing symptoms such as leg pain, heaviness, swelling and tiredness, especially when worn during the day and before getting out of bed in the morning," he said. However, he clarified that current evidence does not show that compression stockings can stop varicose veins from progressing or prevent new veins from developing. "They help manage symptoms but do not treat the underlying problem of faulty vein valves," he explained.
Dr Chaitanya added that lifestyle measures remain an important part of conservative management. According to him, regular walking and calf muscle exercises activate the calf-muscle pump, helping push blood back towards the heart, while avoiding prolonged standing or sitting and elevating the legs above heart level can help reduce swelling and discomfort. "These recommendations are supported by clinical evidence as part of conservative management and are often advised along with compression stockings," he said. At the same time, he stressed that these measures cannot repair damaged vein valves or reverse existing varicose veins.
Addressing home remedies, Dr Chaitanya said there is "no strong scientific evidence" that creams, herbal products or apple cider vinegar can treat or remove varicose veins.
He also advised people not to ignore warning signs that may indicate disease progression. "Patients with persistent pain, swelling, skin changes, bleeding or ulcers should seek medical evaluation," he said, explaining that compression stockings and lifestyle measures alone may no longer be sufficient in such cases. A vascular specialist typically performs a Doppler ultrasound to identify the affected veins, following which minimally invasive procedures such as endovenous laser ablation, radiofrequency ablation, ultrasound-guided foam sclerotherapy and medical adhesive closure can be offered. These procedures, he noted, are performed through small punctures, usually without major surgery, allowing faster recovery while treating the underlying cause of varicose veins and reducing the risk of future complications.
